A 19-year-old female presented with worsening fever, cough and congestion after vaping Juul e-cigarette. Baseline CT Chest reveals ill-defined hazy opacities in the right greater than leftanterior upper lobe, multifocal groundglass opacity noted in the right middle lobe and lingula as well as to a lesser extent at the lung bases. Mild lower lobe predominant interstitial prominence with some air bronchograms seen in the lower lungs. Follow-up CT chest 2 days later shows worsening, extensive consolidation in both lower lobes, left greater than right showing marked progression from the previous exam. There is also significantly increased groundglass opacity within much of the left upper lobe inferiorly. Groundglass opacities also have spread into the lateral portions of the right middle lobe and increased consolidation in the superior segments of both lower lobes. Progressing of diffuse pneumonic process is consistent with a vaping induced autoimmune lipoid pneumonia suspected
